Registration Day for 2015-2016 will be Wednesday August 5, 8 am – 1 pm and 4 pm – 7 pm at Edison.  We'll send details soon, but in the meantime, you can get more information

Attached is a school supply list, just in time for weekend shopping!

The Champaign Public Library hours have changed: the library now closes at 6pm (instead of 9pm) on Thursdays and Fridays.    Please make appropriate arrangements for your middle school students who were planning to use the library after 6 pm on Thursdays and Fridays.

 

Remember that the new deadline to submit completed physical/immunization forms is September 1. Details of the last C-U Public Health school physical clinic

Athletics: attached is a flier listing all our IESA teams and the deadlines for the required paperwork for each team.

Required paperwork: the due date for Baseball and Cross Country forms has passed.  Girls Basketball is due this Friday (July 17) and includes:

   a. Completed Unit 4 form (attached)

   b. Current physical (must be 395 days from date signed to be current) - if your child turned in a physical last year please let me know so I can check the date - attached

   c. Copy of your insurance/medical card

   d. Completed waiver form (attached)

*please do not give the paperwork to the coaches or the main office*

 

Baseball tryout dates - form attached.  Anyone wishing to be considered for the team must attend both days of tryouts.  There will be a parent-player meeting for the selected team on Wed., Aug. 5 beginning at 5:30 pm in the Edison cafeteria.

 

Cross Country: IESA season begins Monday, Aug. 3 - anyone interested in the team must begin practice on this day.  The parent - runner meeting will be Monday, Aug. 3 beginning at 5:30 pm in the Edison cafeteria.

 

Girls Basketball: required paperwork

To:       Parents Enrolling Students in Pre-K, Kindergarten, 1st Grade, 6th Grade and/or 9th Grade

Re:       Health Examinations and Vaccinations

 

On March 9, 2015, the Board of Education approved changes to the District's policy on Health Examinations and Vaccinations.  As part of this policy, the deadline for families to comply with all Health Examination and Vaccination requirements has changed from October 15th of the school year to September 1st of the school year. 

A student's parent(s)/guardian(s) shall present proof that the student received a health examination and received the immunizations against, and screenings for, preventable communicable diseases, as required by the Illinois Department of Public Health, within one year prior to:

 

1.         Entering preschool, kindergarten, or the first grade;

2.         Entering the sixth and ninth grades; and

3.         Whenever a student first enrolls in an Illinois school, regardless of the student's grade (including nursery school, special education, Headstart programs operated by elementary or secondary schools, and students transferring into Illinois from out-of-state or out-of-country).

 

The required health examinations must be performed by a licensed physician, an advanced practice nurse who has a written collaborative agreement with a collaborating physician authorizing the advanced practice nurse to perform health examinations, or a physician assistant who has been delegated the performance of health examinations by a supervising physician.

 

In completing the enrollment process, families are urged to make sure that proof of all required physical examinations and vaccinations is obtained and submitted in a timely manner.  Beginning on September 1st, students who are found to be non-compliant with this policy will be excluded from attending school until all required physical examinations and vaccinations have been completed.
