Edison's Mission

It is our mission at Edison Middle School to work collaboratively to place the focus on the academic, social, and emotional needs of each individual student first and ensure that ALL students are prepared for post-secondary education.

 

Registration for 2015-2016: UPDATE: Please do not use the registration forms that were attached to the previous email/on the Unit 4 web site.  Registration Day is Wednesday August 5, 8 am – 1 pm and 4 pm – 7 pm.  Every student must register; necessary forms will be available at school. Sixth graders and students new to Unit 4 should also bring completed physical and immunization records to registration!

Other handy information about registration:

1.     All parents/guardians need to bring proof of residency which may include an apartment/house lease, mortgage documents, voter's registration, current driver's license, other documents acceptable to administration.
2.     Bring your checkbook or cash. 
3.     Registration fee is $80.
4.     PTSA membership (optional) is $12 or $15 for a membership and car magnet. Your membership dues are greatly appreciated and support PTSA activities that benefit your child's educational experience.
5.     Spiritwear (optional) will also be available for purchase.

Late Registration: if you can't make it to registration on August 5, Late Registration will be Monday August 10 from 10 am – 4 pm at school.

Help with Registration Day: PTSA could use a few friendly folks to assist with registration day activities, including the sale of PTSA memberships/spiritwear and answering general questions about PTSA and school activities.  If you can help, please use the SignUpGenius here www.SignUpGenius.com/go/10C054AA8AA2EA7FB6-registration1 or contact Gretchen Pein at gretchenpein@gmail.com.

The Champaign Public Library hours have changed: the library now closes at 6pm (instead of 9pm) on Thursdays and Fridays.    Please make appropriate arrangements for your middle school students who were planning to use the library after 6 pm on Thursdays and Fridays.

 

Remember that the new deadline to submit completed physical/immunization forms is September 1. Details of the last C-U Public Health school physical clinic

Required paperwork: the due date for Baseball and Cross Country forms has passed.  Girls Basketball is due this Friday (July 17) and includes:

   a. Completed Unit 4 form (attached)

   b. Current physical (must be 395 days from date signed to be current) - if your child turned in a physical last year please let me know so I can check the date - attached

   c. Copy of your insurance/medical card

   d. Completed waiver form (attached)

Baseball tryout dates - form attached.  Anyone wishing to be considered for the team must attend both days of tryouts.  There will be a parent-player meeting for the selected team on Wed., Aug. 5 beginning at 5:30 pm in the Edison cafeteria.

 

Cross Country: IESA season begins Monday, Aug. 3 - anyone interested in the team must begin practice on this day.  The parent - runner meeting will be Monday, Aug. 3 beginning at 5:30 pm in the Edison cafeteria.

On March 9, 2015, the Board of Education approved changes to the District's policy on Health Examinations and Vaccinations.  As part of this policy, the deadline for families to comply with all Health Examination and Vaccination requirements has changed from October 15th of the school year to September 1st of the school year. 

A student's parent(s)/guardian(s) shall present proof that the student received a health examination and received the immunizations against, and screenings for, preventable communicable diseases, as required by the Illinois Department of Public Health, within one year prior to:

 

1.         Entering preschool, kindergarten, or the first grade;

2.         Entering the sixth and ninth grades; and

3.         Whenever a student first enrolls in an Illinois school, regardless of the student's grade (including nursery school, special education, Headstart programs operated by elementary or secondary schools, and students transferring into Illinois from out-of-state or out-of-country).

 

The required health examinations must be performed by a licensed physician, an advanced practice nurse who has a written collaborative agreement with a collaborating physician authorizing the advanced practice nurse to perform health examinations, or a physician assistant who has been delegated the performance of health examinations by a supervising physician.

 

In completing the enrollment process, families are urged to make sure that proof of all required physical examinations and vaccinations is obtained and submitted in a timely manner.  Beginning on September 1st, students who are found to be non-compliant with this policy will be excluded from attending school until all required physical examinations and vaccinations have been completed.

Remember! School Physicals Are Required to Stay in School

The school physical and immunization deadline is now September 1, 2015. In previous years, the deadline has been October 15, so please note this earlier deadline. After September 1 students will be excluded from attending school until all required physical examinations and vaccinations have been completed.

All entering kindergarten, 6th grade, and 9th grade students are required to have proof of a physical and required immunizations. Students entering 12th grade have a vaccine requirement only. Appointment cards will not be accepted as proof of physical and immunizations.

Clinics will be held June 27 and July 25 at C-U Public Health District (201 W. Kenyon Rd.) from 10 a.m. to 1 p.m.
