To: Parents Enrolling Students in Pre-K, Kindergarten, 1st Grade, 6th Grade and/or 9th Grade

 

Re: Health Examinations and Vaccinations

 

On March 9, 2015, the Board of Education approved changes to the District's policy on Health Examinations and Vaccinations.

 

As part of this policy, the deadline for families to comply with all Health Examination and Vaccination requirements has changed from October 15th of the school year to September 1st of the school year. 

A student's parent(s)/guardian(s) shall present proof that the student received a health examination and received the immunizations against, and screenings for, preventable communicable diseases, as required by the Illinois Department of Public Health, within one year prior to:

 

1.     Entering preschool, kindergarten, or the first grade; 

2.     Entering the sixth and ninth grades; and

3.     Whenever a student first enrolls in an Illinois school, regardless of the student's grade (including nursery school, special education, Headstart programs operated by elementary or secondary schools, and students transferring into Illinois from out-of-state or out-of-country).

 

The required health examinations must be performed by a licensed physician, an advanced practice nurse who has a written collaborative agreement with a collaborating physician authorizing the advanced practice nurse to perform health examinations, or a physician assistant who has been delegated the performance of health examinations by a supervising physician.

 

In completing the enrollment process, families are urged to make sure that proof of all required physical examinations and vaccinations is obtained and submitted in a timely manner.  Beginning on September 1st, students who are found to be non-compliant with this policy will be excluded from attending school until all required physical examinations and vaccinations have been completed.
