Required paperwork includes:
1. current physical (if you have a sixth grader and you turned the physical in to the office please let me
know). Phsycial form is attached
2. Completed Champaign Unit 4 form (attached)
3. Copy of your insurance card or medical card
Tryout dates/times for Wrestling and Volleyball will be announced soon.
